-To better serve our students we provide certain forms on-line. -Please complete the following forms by the applicable deadlines:

Half-price Dependent Refund	Term                        Availability Fall                       July 14-Sept 10, 2008 -NOTE: Refunds are processed approximately 2 weeks after the deadline and will be posted to the Dependent's student account.
FACTS Change Form	All adjustments to FACTS must be processed at least five business days prior to the scheduled payment. To request an adjustment, complete this form and fax to (919) 761-2299 or bring it to the Accounting Services Office.
Check Request	Must be received by 12 p.m. Monday or Thursday
