Checklist / Forms

The printable checklist below is for your convience.

     
Application for Admission
     A recent wallet size Photograph (color or black and white)
     Personal Statement explaining your salvation, your commitment and call to ministry, and your reasons for applying to Southeastern.
     Two completed Character Reference Forms from individuals who have known for at least two years (must be non-family members)
     A Pastor Reference Form from an individual that has known you for at least one year. (must be non-family member)
     A $40.00 nonrefundable Application Fee
       - The application fee can be submitted via check or money order. Please include the name of the applicant on the check or money order and mail to the Admissions Office.
     Medical Information Form
     Southeastern Covenant
     Immunization History Form (North Carolina State Health Department standards require that a completed immunization record be received by Southeastern.)
     Official Transcript(s) from all post-secondary schools attended, sent directly to the Admissions Office from the Registrar of each school previously attended.
     Church Recommendation Form
     Spouse’s Personal Statement (if married)
